Cardiovascular Diseases
Cardiovascular diseases are a group of diseases that affect the heart and vascular system, including heart disease and peripheral vascular disease. Heart disease mainly involves abnormalities in the structure of the heart, such as coronary artery disease, which is caused by insufficient blood supply to the coronary arteries leading to myocardial ischemia or hypoxia. Peripheral vascular disease, on the other hand, affects blood vessels other than the heart, such as abnormal blood supply vessels in the upper and lower extremities and trunk.
Targeted therapies against specific receptors and signaling pathways are an important direction in drug development for cardiovascular diseases, e.g:
- G protein-coupled receptors (GPCRs): GPCRs are a large class of cell-surface receptors that regulate a variety of physiological processes in the cardiovascular system, including the control of blood pressure and heart rate. Apelin receptor (APLNR) is a member of the GPCR family, which is potentially therapeutic in cardiovascular disease. By designing specific agonists, it is possible to activate the G protein signaling pathway and reduce deleterious myocardial hypertrophy, providing a new strategy for the treatment of heart failure.
- Thromboxane A2 receptor (TP): TP is a key regulator of platelet aggregation and vascular smooth muscle cell contraction, and is a potential target for the treatment of atherosclerosis and hypertension. By studying the binding mode of TP to its ligands, specific TP inhibitors can be designed to reduce cardiovascular disease.
- Ion channels: Ion channels in cardiac and vascular smooth muscle cells play a central role in regulating cardiac rhythm and vascular tone. Drugs that target specific ion channels, such as calcium channel blockers, have been widely used to treat hypertension and angina.
- Enzyme inhibitors: For example, angiotensin-converting enzyme (ACE) inhibitors and angiotensin receptor blockers (ARBs) lower blood pressure and reduce the risk of cardiovascular disease by blocking the renin-angiotensin system.
- Fatty acid and lipid metabolism modifiers: Statins lower cholesterol levels and reduce the risk of cardiovascular disease by inhibiting HMG-CoA reductase.
- Anticoagulant drugs: Prevent thrombosis by reducing blood clotting and are used to treat or prevent stroke and myocardial infarction.
