Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by aberrant immune system activation, multi-organ involvement, and autoantibody production. Its etiology involves multifactorial contributions from genetic, environmental, and hormonal factors. Core pathological mechanisms include dysregulated activation of B cells leading to autoantibody production, T cell dysfunction, aberrant activation of interferon signaling pathways, and complement system dysregulation. Conventional therapies rely on glucocorticoids and immunosuppressants but carry risks of infection and organ toxicity, underscoring the need for targeted precision therapies.
Recent advances in SLE drug development focus on modulating key immune checkpoints. Belimumab, the first FDA-approved B cell activating factor (BAFF) inhibitor, blocks BAFF/APRIL signaling to suppress B cell maturation. Telitacicept, a recombinant fusion protein, neutralizes both BAFF and a proliferation-inducing ligand (APRIL), enhancing B cell depletion. Anifrolumab targets the type I interferon receptor, blocking excessive interferon signaling activation. JAK inhibitors such as baricitinib regulate T cell and plasma cell functions by inhibiting the JAK-STAT pathway. Upadacitinib, a selective JAK1 inhibitor, further optimizes efficacy-safety profiles. Emerging approaches include belimumab's new indication targeting complement C1s and CAR-T cell therapy demonstrating promise in refractory SLE, marking a shift from single-target to multi-pathway combination strategies.
