Crohn's Disease

Crohn's Disease (CD) is a chronic inflammatory bowel disease, a type of inflammatory bowel disease. It can affect any part of the digestive tract, from the mouth to the anus, but is most common in the end of the small intestine (ileum) and the large intestine (colon). It usually presents as a segmental inflammation of the gastrointestinal tract and may cause symptoms such as abdominal pain, diarrhea, rectal bleeding, weight loss, fever and fatigue.

The current treatment strategy for Crohn's disease focuses on the use of medications to control inflammation and prompt surgical intervention to alleviate the symptoms of intestinal injury, with drug development focusing on the following directions:

• Anti-TNF drugs: Infliximab and Adalimumab are widely used anti-TNF drugs, and they show good efficacy and safety in the treatment of Crohn's disease.
• JAK Inhibitors: Upadacitinib and Tofacitinib are new treatment options, especially Upadacitinib has been approved for the treatment of moderate to severe Crohn's disease and has shown significant efficacy and safety.
• IL-23 inhibitors: Guselkumab and Risankizumab target the p19 subunit of IL-23 and are effective in inducing and sustaining remission, and have been shown to have positive results in clinical trials.
• CCR9 antagonist: CCX282-B is a novel drug that inhibits the intestinal inflammatory response by targeting the CCR9 receptor and is currently in preclinical studies.
• α4β7 integrin inhibitor: Vedolizumab has been approved by the FDA for the treatment of Crohn's disease, showing favorable efficacy and safety.

Other novel drugs in development include those targeting the IL-12/23 axis and those based on gene therapy, which are expected to provide more treatment options for Crohn's disease patients.
