World Sickle Cell Day

Every year on June 19th, World Sickle Cell Day aims to promote support for patients and families, advance medical research and education to improve the quality of life for individuals with sickle cell disease, and raise public awareness and understanding of the condition. This year's theme is "Hope Through Progress: Advancing Care Globally."

Sickle cell disease is a genetic blood disorder that primarily affects the shape and function of red blood cells. Under normal circumstances, red blood cells are round with a concave center, flexible like rubber bands, and able to move through blood vessels to deliver oxygen throughout the body. However, in individuals with sickle cell disease, a mutation in the hemoglobin molecule causes the red blood cells to deform into crescent or sickle shapes, making them rigid and prone to breakage. Not only are these altered cells ineffective at transporting oxygen, they can also block blood vessels, leading to painful crises, organ damage, anemia, and various infections.

The fundamental cause of sickle cell disease lies in a mutation of the beta chain of hemoglobin, resulting in the formation of hemoglobin S (HbS). A minor change in the sixth amino acid of the β chain from glutamic acid to valine in normal hemoglobin (HbA) leads to abnormal polymerization between hemoglobin molecules, causing the deformation of red blood cells.

In recent years, research on sickle cell disease has encompassed multiple areas, including gene therapy, drug treatment, and stem cell therapy. Gene therapy, as one of the forefront explorations, seeks to directly correct faulty gene sequences using gene editing techniques such as CRISPR-Cas9, or activate the production of fetal hemoglobin (HbF) to restore normal red blood cell function. In addition, a range of innovative drugs such as hydroxyurea, L-glutamine, and voxelotor alleviate symptoms through different mechanisms, improving the quality of life for patients. While stem cell transplantation carries risks and has limited applicability, it is still considered a potential curative approach, especially when combined with the latest advances in cell therapy, bringing new hope for patients.
